Ella Rises Summit — Participant Agreement & Consent

Welcome to the Ella Rises Summit! We're so glad you and your family are joining us. Before the day begins, we ask every adult registering to read and agree to the few things below. They help us keep everyone safe, protect the young women in our care, and make sure you know exactly how we'll treat your information. Thank you for taking a moment with us.

 

Organization: Ella Rises Foundation ("Ella Rises," "we," or "us")

Event: Ella Rises Summit and related activities -- August 10, 2026, at Thanksgiving Point

 

By registering for or attending the Event, the adult signing below ("I" or "Parent/Guardian") agrees to the following for themselves and for any minor they are registering ("Participant").

 

1. Taking part, and the risks we accept:

Joining the Summit is voluntary, and like any in-person gathering it carries some normal risks — physical activity, group activities, using the venue and its equipment, getting to and from Thanksgiving Point, being around other people, and ordinary exposure to illness. I understand these risks can't be fully removed, and knowing that, I choose for myself and my Participant to take part and accept those risks.

2. Releasing Ella Rises from liability:

 

To the fullest extent allowed by law, I — for myself, my Participant, and our families and representatives — release and hold harmless Ella Rises Foundation and its directors, officers, employees, volunteers, instructors, sponsors, community partners, and the owners of the Thanksgiving Point venue from any claim for injury, illness, loss, or property damage arising from participation in the Event, including any claim based on their ordinary negligence. This release does **not** apply to gross negligence or intentional misconduct.

 

3. Harm caused to others:

​

 

If I or my Participant cause injury to another person or damage to someone's property, or break the Event rules, I agree to be responsible for those costs, to the extent the law allows.

 

4. If there's a medical emergency

 

If someone is hurt and I can't be reached, I authorize Ella Rises staff and volunteers to arrange emergency medical care for me or my Participant, and I accept responsibility for those costs. I confirm the Participant is able to take part, and I've shared anything important in the registration page including emergency contact name, phone, and any allergies or medical conditions.

5. Photos, video, and stories from the day:

 

Photographs, video, audio, and interviews are part of how we capture and share the Ella Rises experience. I give Ella Rises permission to record and use the image, likeness, voice, and words of myself and my Participant — on our website, social media, and in print, promotional, educational, and fundraising materials — without payment.

(To protect the young women we serve, we do not publish minors' full names alongside their images.)

That's completely okay. You can opt out on the registration form.

6. Your privacy and your information

 

We only collect what we need to run the Summit and our programs — names, contact details, dates of birth, and t-shirt sizes. Here's our promise:

 

• Your information belongs to you and to Ella Rises. We will not sell, rent, or trade it to third parties for their marketing.

• We share it only with the services that help us run the event (for example, our payment processor for your $10 registration), and only as much as they need.

• We keep it only as long as we need it for our programs and records, and we protect it with reasonable safeguards.

• You can ask to see, correct, or delete your information anytime at info@ellarises.org.

7. Kindness and respect:

 

We ask everyone to be respectful and to follow the guidance of Ella Rises staff and volunteers. We may ask anyone whose behavior is unsafe or disruptive to leave, and registration in that case isn't refundable.

 

8. Signing on behalf of a minor:

 

If I'm registering anyone under 18, I confirm I am that young woman's parent or legal guardian, that I have the authority to agree on her behalf, and that I give permission for her to take part and agree to everything here for her.

 

9. The fine print:

 

This agreement is governed by the laws of the State of Utah, with any dispute handled in the courts of Utah County. If any part of this agreement can't be enforced, the rest still stands. By submitting this form online and checking the box, I'm providing my legal signature, the same as signing by hand. This is our complete agreement on these matters.

 

How to register and sign:

To take part, complete the Ella Rises Summit registration form and check the box agreeing to this waiver. That check is your electronic signature, and your registration is automatically dated the moment you submit it. On the form you'll also share your emergency and medical information, and you can choose whether to opt out of photos or to receive news about future events.

¡Bienvenida a la Cumbre Ella Rises! Nos alegra muchísimo que usted y su familia nos acompañen. Antes de que comience el día, le pedimos a cada adulta que se inscribe que lea y acepte los siguientes puntos. Nos ayudan a mantener a todas seguras, a proteger a las jóvencitas a nuestro cuidado y a que usted sepa exactamente cómo trataremos su información. Gracias por tomarse este momento con nosotras.

Al inscribirse o asistir al Evento, la adulta que completa el formulario de inscripción ("yo" o "Padre/Madre/Tutor(a)") acepta lo siguiente en nombre propio y de cualquier menor que esté inscribiendo ("Participante").

1. Participar y los riesgos que aceptamos

Participar en la Cumbre es voluntario y, como cualquier reunión presencial, conlleva ciertos riesgos normales — actividad física, actividades en grupo, el uso de las instalaciones y el equipo, el traslado hacia y desde Thanksgiving Point, la convivencia con otras personas y la exposición habitual a enfermedades. Entiendo que estos riesgos no pueden eliminarse por completo y, sabiéndolo, decido que mi Participante y yo tomemos parte y aceptamos esos riesgos.

2. Liberar de responsabilidad a Ella Rises

​

En la máxima medida que permita la ley, yo —por mí misma, por mi Participante y por nuestras familias y representantes— libero y eximo de responsabilidad a Ella Rises Foundation y a sus directores, funcionarios, empleados, voluntarios, instructores, patrocinadores, socios comunitarios y a los propietarios del lugar Thanksgiving Point, de cualquier reclamo por lesión, enfermedad, pérdida o daño a la propiedad que surja de la participación en el Evento, incluido cualquier reclamo basado en su negligencia ordinaria. Esta liberación no aplica a la negligencia grave ni a la conducta intencional.

3. Daños causados a otras personas

​

Si mi Participante o yo causamos una lesión a otra persona o un daño a la propiedad ajena, o incumplimos las reglas del Evento, acepto hacerme responsable de esos costos, en la medida en que lo permita la ley.

4. Si hay una emergencia médica

​

Si alguien resulta herido y no se me puede localizar, autorizo al personal y a los voluntarios de Ella Rises a gestionar atención médica de emergencia para mí o para mi Participante, y acepto la responsabilidad de esos costos. Confirmo que la Participante está en condiciones de tomar parte, y he compartido la información importante en la página de inscripción, incluyendo el nombre y teléfono de un contacto de emergencia y cualquier alergia o condición médica.

5. Fotos, video e historias del día

​

Las fotografías, el video, el audio y las entrevistas son parte de cómo capturamos y compartimos la experiencia de Ella Rises. Doy permiso a Ella Rises para grabar y usar la imagen, semejanza, voz y palabras mías y de mi Participante —en nuestro sitio web, redes sociales y en materiales impresos, promocionales, educativos y de recaudación de fondos— sin pago alguno. (Para proteger a las jóvencitas a quienes servimos, no publicamos los nombres completos de las menores junto a sus imágenes.)

¿Prefiere no aparecer en fotos? Está totalmente bien. Puede indicarlo en el formulario de inscripción.

6. Su privacidad y su información

​

Solo recopilamos lo que necesitamos para realizar la Cumbre y nuestros programas — nombres, datos de contacto, fechas de nacimiento y tallas de camiseta. Esta es nuestra promesa:

• Su información les pertenece a usted y a Ella Rises. No la venderemos, alquilaremos ni intercambiaremos con terceros para su mercadeo.

• La compartimos únicamente con los servicios que nos ayudan a realizar el evento (por ejemplo, nuestro procesador de pagos para su inscripción de $10), y solo en la medida que ellos necesiten.

• La conservamos solo el tiempo que la necesitemos para nuestros programas y registros, y la protegemos con medidas de seguridad razonables.

• Puede solicitar ver, corregir o eliminar su información en cualquier momento escribiendo a info@ellarises.org.

7. Amabilidad y respeto

​

Pedimos a todas que sean respetuosas y que sigan las indicaciones del personal y los voluntarios de Ella Rises. Podemos pedir que se retire a cualquier persona cuyo comportamiento sea inseguro o disruptivo, y en ese caso la inscripción no es reembolsable.

8. Firmar en nombre de una menor

​

Si estoy inscribiendo a alguien menor de 18 años, confirmo que soy su padre, madre o tutor(a) legal, que tengo la autoridad para aceptar en su nombre, y que doy permiso para que tome parte y acepto todo lo aquí expuesto en su nombre.

9. La letra pequeña

​

Este acuerdo se rige por las leyes del Estado de Utah, y cualquier disputa se resolverá en los tribunales del Condado de Utah. Si alguna parte de este acuerdo no pudiera hacerse cumplir, el resto permanece vigente. Al enviar este formulario en línea y marcar la casilla, proporciono mi firma legal, con el mismo valor que firmar a mano. Este es nuestro acuerdo completo sobre estos asuntos.

Cómo inscribirse y firmar
